Polk County Health Services is a private-not-for-profit agency located in Des Moines, Iowa. It is responsible for county-wide planning, program recommendation, implementation, and evaluation of services for persons with mental illness, mental retardation, developmental disabilities, and chemical dependency. The purpose of this project is for LHPDC to evaluate services provided in the county and report these evaluations to PCHS annually.
