State Resources for Disability Program Navigators to Coordinate Systems Change Activities

The following information on systems change grantees was put together by the Law, Health Policy & Disability Center (LHPDC), TA and Evaluation Provider for the 14 Disability Program Navigator states. It is hoped that this contact information will be useful to the Navigators as they begin making connections with other disability-related grantees within their state.
