Performance Provider Profile and Report Card

Through its employment project with Polk County Health Services (Des Moines, Iowa) the Law, Health Policy and Disability Center and its partners have developed a Performance Provider Profile and Report Card. The profile and report card is designed to provide consumers with key information to help them choose their supported employment provider.

The first part of the report, called the Performance Provider Profile, describes the services that each employment service provides. The second part of the report, called the Performance Report Card, compares the employment service providers on five key outcome measures.

To assist consumers and their representatives to use the Performance Provider Profile and Report Card, a How to Read The Guide information sheet was developed. This guide gives a brief description of the categories in the report and the ways for the reader to analyze the information.

Organizations that assisted the Center in the development of the Provider Profile and Report Card included the National Results Council, Polk County Health Services, the Governor's Developmental Disabilities Council, Community Support Advocates, Golden Circle Behavioral Health Care, and the Ankeny Chamber of Commerce.
